GPs assess patients who may be due for a Home Medicines Review. Age, number of medicines, recent hospital discharge, side-effects or poor response to treatment are key indicators.
The GP completes an HMR referral via our secure online form (integrated with the PMS). Patient and clinical details are sent safely to the accredited pharmacist.
The pharmacist meets the patient, reviews all medications, storage and administration, assesses risk and provides support and guidance.
The GP receives the clinical report, reviews findings with the patient and updates the medication management plan as required.
The GP bills Medicare Item 900. Ongoing collaboration and follow-up are supported through the DMMR Partners portal.
