A Home Medicines Review (HMR / DMMR) helps you understand your medicines, reduce medication risks, and feel more confident managing your treatment — with support from your GP and an accredited pharmacist.
A Home Medicines Review is a service where an accredited pharmacist works with your GP to review all your medicines — including prescriptions, over-the-counter medicines, vitamins and supplements.
The goal is to make sure your medicines are safe, effective, and appropriate for your health needs, while helping prevent confusion and side effects.
Your GP may recommend an HMR if you:
Your GP will decide if a review is clinically appropriate.
Your GP recommends a review and refers you to an accredited pharmacist.
The pharmacist meets with you to discuss your medicines and any concerns.
The pharmacist prepares a report and shares it securely with your GP.
Your GP reviews recommendations and updates your treatment plan if needed.
Your information is handled securely and is only shared between your GP and the accredited pharmacist providing your review.
The purpose of the review is to support your health and ensure your medicines are safe and appropriate.
You may ask questions or decline the service at any time.
