What is a Home Medicines Review?

A Home Medicines Review is a service where an accredited pharmacist works with your GP to review all your medicines — including prescriptions, over-the-counter medicines, vitamins and supplements.

The goal is to make sure your medicines are safe, effective, and appropriate for your health needs, while helping prevent confusion and side effects.

Who is Eligible for a Home Medicines Review?

Your GP may recommend an HMR if you:

take five or more regular medicines
have recently changed or started new medicines
were recently discharged from hospital
are feeling unwell or experiencing side effects
have difficulty remembering or managing medicines
live with multiple or complex health conditions

Your GP will decide if a review is clinically appropriate.

What Can I Expect During a Home Medicines Review?

Step 1

GP Referral

Your GP recommends a review and refers you to an accredited pharmacist.

Step 2

Pharmacist Appointment

The pharmacist meets with you to discuss your medicines and any concerns.

Step 3

Medication Report

The pharmacist prepares a report and shares it securely with your GP.

Step 4

Follow-Up With Your GP

Your GP reviews recommendations and updates your treatment plan if needed.

Your information is handled securely and is only shared between your GP and the accredited pharmacist providing your review.

The purpose of the review is to support your health and ensure your medicines are safe and appropriate.
